We are out of network with all health plans and we are not enrolled as Medicare providers. Therefore, private payment is required at time of service. If you have out of network benefits from your commercial insurance plan (not including Medicare), we will give you a statement that you can submit to your health plan for services your plan covers. We cannot provide Medicare patients with a statement to submit for reimbursement since we are not enrolled Medicare providers.
